“Cholesterol pills”: precautions!
First of all, it is definitely worth noting that virtually any “cholesterol pills” are not at all medications that you can “treat” yourself! Remember, the main purpose of this article is to simply (briefly) introduce you to all the main drugs (indirectly or purposefully) that reduce high cholesterol in the blood.

Nicotinic acid preparations for lowering cholesterol levels
Nicotinic acid preparations for lowering cholesterol contain B-complex vitamins. Nicotinic acid, or niacin, is found in food, but to achieve faster results, it is better to take it in the form of medications prescribed by a specialist.

- Common side effects of statins include arthralgia (i.e., intermittent joint pain), dyspepsia (i.e., stomach upset; difficult/painful digestion), constipation, heaviness, and abdominal pain (most commonly in the epigastric region);
- the next common problem is the symptoms of myositis (i.e., inflammatory damage to the skeletal muscles), but very rarely they develop directly into a “real” disease (1 case in (!) 2000 patients);
- EXTREMELY RARE cases of the development of severe myopathy, rhabdomyolysis (which are accompanied by quite severe muscle pain), as well as serious liver damage and an “abnormal” increase in sugar levels have been observed;
- and, finally, in the RAREEST cases - these pills (medicines) can cause short-term “memory lapses” (although complaints were received ONLY from patients from the USA), but they disappear - in fact immediately after stopping taking the drugs (a very strange phenomenon, but the Americans “fixed” it).
Contraindications and restrictions:
- statins are strictly contraindicated during pregnancy (or during breastfeeding), as well as for people with severe (chronic) liver diseases;
- for elderly men and women, it is recommended that very careful treatment be started, starting with minimal doses of statins (since it is quite possible that there is increased sensitivity to them);
- some statin medications may interact poorly with other medications (BE SURE to tell your doctor which specific pills you are already taking);
- DO NOT FORGET that grapefruit juice or grapefruit itself can significantly increase the effect of Lovastatin, Simvastatin and Atorvastatin!

Citrus fruits, black currants, white cabbage, rose hips - are characterized by a high content of ascorbic acid, which reduces tissue permeability and prevents the penetration of cholesterol into the vascular walls. It is an antioxidant, slows down the oxidation of vitamin C in tissues and, by reducing their permeability, reduces the rate of excretion of vitamin C from the body.
Most rutoside is found in chokeberry, black currant, rose hips, oranges and lemons.
We must not forget that citrus fruits, rose hips and black currants are contraindicated for gastritis, stomach and duodenal ulcers with high acidity. It is not recommended to use these products if you have increased blood clotting or allergies. White cabbage will have to be abandoned in case of diarrhea, myocardial infarction, acute phase of colitis and gastritis, as well as flatulence.
It should be remembered that ascorbic acid serves only as a prophylactic against atherosclerosis. If there are already plaques in the vessels, ascorbic acid alone will not get rid of them.

Dioscorea preparations should not be taken for bradycardia, hypotension, exacerbation of gastritis and gastric and duodenal ulcers. It is recommended to drink infusions and decoctions of the plant after meals, since the substances contained in Dioscorea can irritate the gastric mucosa.
To prepare a healing infusion, pour 1 teaspoon of dioscorea root powder into 250 ml of boiling water, leave for 20 minutes, strain, mix with 1 teaspoon of honey. Bile acid sequestrants (BAS)
Bile acid sequestrants (“binders”) (or “ion exchange resins”) are a group of drugs that for several decades (before the advent of statins) WAS one of the most popular drugs for reducing the level of “bad” LDL cholesterol in the blood (by 15 -thirty%). Which, of course, reduced the risks of developing IHD (Coronary Heart Disease), as well as other cardiovascular diseases. However, they had no effect on the amount of beneficial HDL cholesterol in the blood and INCREASED (!) the concentration of triglycerides.
In addition, bile acid sequestrants (BAS) had a high frequency of side effects (in particular, constipation in (!) 50% of cases). And also (according to “Soviet” statistics) many patients “flatly” refused to use these medications due to fairly/unpleasant taste sensations. And finally, the biggest problem was that these drugs seriously interfered with the absorption of most other life-saving/essential medications (diuretics, beta blockers, etc.). Perhaps this is why this group of drugs is NOT (!) ALLOWED for use in the Russian Federation.
Bile acid sequestrants are now used only abroad and only in the most extreme cases...
At the moment (when many more advanced lipid-lowering drugs have appeared, with a minimal list of side effects), FFAs (in particular, Colesevelam / Welchol) are prescribed (only!) in combination therapy with statins (and only abroad!) for obtaining an additional effect when reducing dangerously/high levels of LDL cholesterol in the blood in patients suffering from heterozygous hereditary (familial) hypercholesterolemia.

- stomach and duodenal ulcers;
- disorders of coagulation and fibrinolysis;
- malabsorption syndrome;
- biliary cirrhosis of the liver;
- problems with constipation (and hemorrhoids).
Fibrates to lower blood cholesterol
These cholesterol-lowering drugs affect lipid metabolism. If we compare medications from this group with statins, they act on sterols of different densities. Fibrates effectively affect high-density cholesterol, statins – low-density cholesterol. In terms of popularity in medicine, fibrates are significantly inferior to statins; they are used less frequently in solving this problem.
Indications for the use of fibrates are:
- triglyceride level from 4.5 mmol/l;
- isolated hypoalphacholesterolemia in people suffering from coronary heart disease;
- decrease in HDL cholesterol levels.
The use of fibrates to reduce the level of sterol in the blood allows patients to avoid the development of pancreatitis and reduce the risks of cardiovascular complications. They also activate the process of producing beneficial sterol.
However, improper use of medications to lower blood cholesterol, which are fibrates, can cause a number of side effects. Treatment may be accompanied by nausea, vomiting, pain in the abdominal area, diarrhea, and flatulence.
Fibrates include the following medications:
- Clofibrate;
- Bezafibrate;
- Fenofibrate;
- Gemfibrozil;
- Ciprofibrate.
It is strictly forbidden to take fibrates and statins at the same time, since this combination can lead to quite dangerous consequences.
Cholesterol absorption inhibitors (ezetimibe, ezetrol, zetia)
Cholesterol absorption inhibitors are lipid-lowering drugs (“EZETIMIB” / “EZETROL” tablets) that “inhibit the absorption” (i.e., “prevent the absorption”) of cholesterol in the human intestine (small intestine). ) together with statins for the treatment of primary hypercholesterolemia.
Cholesterol absorption inhibitors are quite effective in reducing elevated levels of Total Cholesterol in the blood, LDL cholesterol, apolipoprotein B, triglycerides, and also increase the amount of useful HDL cholesterol. However, in the treatment of homozygous hereditary hypercholesterolemia, it may be necessary to use other (additional) agents, for example, such as LDL apheresis.

EZETIMIBE – side effects (actions):
The most common side effects with monotherapy (ezitimibe or other cholesterol absorption inhibitors) are headaches, abdominal pain and diarrhea. In combination treatment with statins: headaches, fatigue, abdominal pain, constipation, diarrhea, bloating, nausea, increased AST and ALT activity, myalgia. Extremely rare side effects are myopathy and rhabdomyolysis.
Contraindications and restrictions:
- This medicine is not prescribed to patients with moderate (or severe) liver failure, because in this case, its concentration in the blood plasma increases significantly;
- Ezetimibe tablets (at least for now) are NEVER (!) prescribed together with fibrates (medicines to reduce elevated triglyceride levels in the blood);
- "Ezetimibe" is contraindicated for people with hypersensitivity to any of the components of this drug, children and adolescents under 18 years of age, as well as for pregnant (or lactating) women;
- is prescribed with extreme caution (by doctors) for patients taking Cyclosporine (when using this combination, blood serum levels should be monitored).
The effect of dietary supplements on cholesterol levels
All supplements have almost the same effect on the body. The main component of each dietary supplement is plant raw materials. Such cholesterol-lowering pills will have to be taken for quite a long time, but they have significantly fewer unwanted effects, and some have none at all.

According to research results, the “PCSK9 inhibitor” is able to (record) reduce the level of “bad” LDL cholesterol in the blood by 50-60%! Injections are given once every 2 weeks (the injection is administered subcutaneously). The medicine has received a lot of grateful reviews, but it has a simply “cosmic” cost: from 200 euros per package (with one filled “syringe/pen”)!

These medications are produced in tablet, capsule and powder forms. They are easy to use - just drink them with a glass of distilled water. If cholesterol levels are high in the lipid profile, monotherapy with dietary supplements is not indicated - dietary supplements should only be an addition to the main drug treatment.
Policosanol (from cane sugar)
Policosanol is a popular dietary supplement that a doctor can prescribe as an addition to the main therapy of atherosclerosis. The main active ingredient of the drug is a compound obtained from cane sugar. It affects fat metabolism and metabolism, accelerating and stabilizing them. Fish oil and Omega-3
The main goal for high blood cholesterol is to reduce the level of low-density lipoproteins and increase the concentration of HDL (high-density lipoprotein). Omega-3 fatty acids have this effect. They are a type of polyunsaturated fat. Our body does not produce them on its own, so we can only get them from food.
High content of Omega-3 and Omega-6 in seafood, and especially in fish oil. It is most often sold in capsules and contains EPA and DHA - eicosapentaenoic and docosahexonic acids. In addition to reducing the level of harmful cholesterol, these active compounds also affect the level of triglycerides, reducing it to physiological norms.
Bempedoic acid (ATP citrate lyase inhibitor)
These are the newest “cholesterol pills 2019-2022”, however (!) at the moment – they are not yet (!) approved by the FDA and EMA. The estimated date of entry into the pharmaceutical market is 2022. Their most important advantage is the significant assistance they provide to those patients (in lowering LDL cholesterol) who CANNOT take statins - either due to some side effects (for example, muscle pain, in almost 90% of cases), or because of “ incompatibility” of the latter with some other drugs.
According to the results of a series of studies (phase 3 of which was completed in 2020), bempedoic acid (or an ATP-citrate lyase inhibitor) when combined with Ezetemibe reduces LDL cholesterol levels by 43%. That is why the company has submitted an application to the FDA and EMA - for now, to produce tablets with bempedoic acid, and to take it exclusively in combination with Ezetemibe.
The medicine will be taken only once a day, but the cost is by no means cheap (at least by our domestic standards). Specialists from the commercial department of Esperion Therapeutics (i.e., the developer of this drug) suggest that treatment with this newest drug will cost Americans NO MORE than $4,000 per year. Probably, for many, this “NO MORE” will not reassure you at all!
Fibrates (fibric acid derivatives)
Fibrates are highly effective medications designed to reduce elevated triglyceride levels in the blood (up to 50%). Typically, they are prescribed (by doctors) for patients suffering from hypertriglyceridemia, familial combined dyslipidemia, and diabetic dyslipidemia. In addition, fibrates slightly lower LDL cholesterol (by 10-20%), but significantly increase the amount of “good” HDL cholesterol (up to 20-25%)! There are many other beneficial properties of these drugs.
Specifically, to slow the development of atherosclerosis in patients with IHD (Coronary Heart Disease), treating specialists prescribe Bezafibrate or Gemfibrozil. And for patients suffering from type 2 diabetes mellitus - Fenofibrate. Together with statins (cautiously, and in minimal dosages), fibrates are prescribed (!) only for the treatment of isolated hypertriglyceridemia (i.e. endogenous type IV HLP / elevated VLDL levels). Since these drugs work great, but paradoxically increase Total Cholesterol. That is why Simvastatin (10 mg per day) or Fluvastatin (20 mg per day) successfully solve this problem.

By and large, fibrates are tolerated NORMALLY! Although, 5-10% of patients may still experience side effects: abdominal pain, constipation, diarrhea, flatulence, skin rash, insomnia. - vomiting, increased gas formation, gastric and intestinal dyspepsia, diarrhea, abdominal pain, nausea;
- pancreatitis, hepatitis, cholelithiasis;
- myopathy (up to the development of rhabdomyolysis), myalgia, myositis, paresthesia, lower back pain, arthritis;
- migraine;
- impotence, frigidity, infertility;
- thromboembolism of the pulmonary artery and veins;
- dermatological problems.
To lower blood cholesterol levels, combinations of statins and fibrates are possible. At the same time, the dosage of both is reduced, as is the risk of side effects.
Contraindications include pathologies of the liver, gall bladder, and impaired hematopoiesis.
OMEGA 3 (PUFA / Polyunsaturated Fatty Acids)

Niacin (vitamin B3/PP, nicotinic acid, nicotinamide)
In large dosages (much higher than the daily norm), NIACIN reduces the levels of LDL cholesterol (by 20-30%), triglycerides (by 20-50%), and apolipoprotein A (up to 30%).
